This is a program designed to help you stay on top of your health by providing an in-home health and wellness visit by an advanced practice provider.
Our house call option provides care for patients where they live, whether it is at home, nursing home, assisted living, or hospice facility. Coming to your home helps us keep in touch with our patients and maintain continuity of care even for those who cannot come to the office. Coming to you helps us learn more about the patient and connect with them on a more personal and professional level. This option could be done through telehealth as well.
